ABSTRACT

Introduction. Acute stress is related to the energetic release of catecholamines, cytokines, and cortisol, which trigger pain perception. Chewing stimulates serotonergic neurons, these neurons modulate the neural pathways of pain; however, the relationship between these variables is still in the process of understanding. Objective. To determine the effect of acute stress and chewing stimulus on pain. Methods. an experiment that was developed in the Faculty of Medicine of the UNMSM. 40 eight-week-old male Balb/c mice were randomly assigned into 5 equal groups. A: week 1 without any stimulus and week 2 chewing stimulus + stress. B: week 1 without any stimulus and week 2 stress. C: week 1 stress and week 2 without any stimulus. D: week 1 masticatory stimulus + stress and week 2 without any stimulus. E: control, without any stimulus. Pain was assessed in the 5 groups at 7 and 14 days using the tail-withdrawal time test before thermal stimulation. Results. The ANOVA test was used for the intergroup evaluation, finding no significant difference. Using the t student test for related samples, the intragroup evaluation was made, where a significant difference was found between 7 vs. 14 days, both in group A (p=0.029) and in group C (p=0.03). Conclusion. The effect of acute stress on pain perception was decreased by chewing stimulus in Balb/c mice.

ABSTRACT

Introduction. Acute stress alters memory and spatial learning and the expression of interleukin 6, the chewing stimulus would prevent these effects. Objective. To determine the effect of chewing stimulation and acute stress on the expression of interleukin 6 and memory and spatial learning in mice. Methods. Experiment where 70 male albino mice of the Balb/c of age 2 month were randomly distributed into: Group A1: acute stress 1 hour; Group A2: acute stress 1 hour + chewing stimulus 1 hour; Group B1: acute stress 2 hours; Group B2: acute stress 2 hours + chewing stimulus 2 hours; Group C1: acute stress 3 hours; C2: acute stress 3 hours + chewing stimulus 3 hours; Group D: without intervention. For 3 days, spatial memory and learning were tested in the Morris water maze. Interleukin 6 (IL-6) was analyzed by ELISA test. Results. IL-6 was higher in the B2 group vs the other groups (p<0.0001). In addition, on the first day of evaluation, the acquisition of spatial memory and spatial was lower in the A1 vs. A2 group (p=0.042). Conclusión. Only on the first day of evaluation, we found that the masticatory stimulus prevented the decrease in memory acquisition and spatial learning in mice subjected to low-intensity acute stress. The results were generally inconclusive on the effect of masticatory stimulation. In addition, IL-6 was higher in the stress + masticatory stimulus (group B2) over the rest.

ABSTRACT

Objective:To investigate the correlation between stress hyperglycemia ratio (SHR) and poor outcome of successful recanalization after endovascular treatment in patients with acute ischemic stroke. Methods:From January 2019 to October 2022, patients with acute ischemic stroke received endovascular treatment and successful recanalization in the Second People’s Hospital of Lianyungang were included retrospectively. SHR was defined as the fasting blood sugar and glycosylated hemoglobin ratio. At 90 d after procedure, the outcome of patients was evaluated using the modified Rankin Scale score. 0-3 was defined as good outcome, and >3 was defined as poor outcome. Multivariate logistic regression analysis was used to determine the independent risk factor for poor outcome. Receiver operating characteristic (ROC) curve was used to evaluate the predictive value of SHR for 90 d poor outcome in patients with successful recanalization after endovascular treatment. Results:A total of 159 patients were enrolled, including 98 males (61.6%), aged 69.8±8.9 years old. The baseline National Institutes of Health Stroke Scale (NIHSS) score was 12.6±4.3, and SHR was 1.17±0.46. One hundred and five patients (66.0%) had good outcome, while 54 (34.0%) had poor outcome. There were statistically significant differences in SHR, fasting blood glucose, glycosylated hemoglobin, baseline NIHSS score and the proportion of patients with poor collateral circulation and symptomatic intracranial hemorrhage between the poor outcome group and the good outcome group (all P<0.05). Multivariate logistic regression analysis showed that SHR was an independent predictor of poor outcome (odds ratio 2.254, 95% confidence interval 1.136-4.278; P<0.001). The ROC curve analysis showed that the area under the curve of SHR for predicting poor outcome was 0.726 (95% confidence interval 0.648-0.804; P<0.001), which was higher than fasting blood glucose and glycosylated hemoglobin. The optimal cutoff value for SHR was 1.21, and the sensitivity and specificity for predicting poor outcomes were 66.23% and 75.82%, respectively. Conclusion:SHR is associated with the poor outcome of successful recanalization after endovascular treatment in patients with acute ischemic stroke and can be used as a potential predictor.

ABSTRACT

Temporomandibular disorders are frequent in different segments of the population and harm the quality of life of individuals. The present sudy aimed to investigate the association between stress and temporomandibular dysfunction (TMD) in university students in Brazil. All incoming students at the Federal University of Pelotas were invited to participate in this cross-sectional study. A self-administered questionnaire was used with socioeconomic and oral health variables. TMD Fonseca questionnaire "Fonseca's anamnestic index" was used to identify the severity of symptoms of temporomandibular dysfunction. The same was elaborated in the form of Helkimo's anamnestic index. Stress was measured using a modified version of the Perceived Stress Scale and categorized into quartiles. Logistic multivariable regression models were used to analyze the associations of interest controlling for possible confounding variables. A backward stepwise procedure was used to select variables that should be kept in the final model. A total of 2,089 students answered the questionnaires and 82 (3.9%) were classified with the presence of TMD. Adjusted logistic regression shows that chance of presenting temporomandibular dysfunction was higher (OR=2.43; 95%CI=1.04-5.65) when the stress level increased. In conclusion, the mild degree of temporomandibular dysfunction was the most prevalent. Stress was associated with individuals with a higher prevalence of temporomandibular dysfunction. (AU)

(AU)

ABSTRACT

Objective:To investigate effect of blood glucose on early neurological deterioration (END) and outcome after intravenous thrombolysis in patients with acute ischemic stroke (AIS).Methods:Patients with AIS treated with intravenous thrombolysis in the Department of Neurology, the Second Affiliated Hospital of Xuzhou Medical University from June 2020 to December 2021 were collected retrospectively. Demographic and baseline clinical data of the patients were collected. END was defined as an increase of ≥2 in reassessing the maximum score of the National Institutes of Health Stroke Scale within 72 h after admission compared with the baseline. Poor outcome was defined as the modified Rankin Scale score ≥3 at 3 months after onset. Multivariate logistic regression analysis was used to evaluate the independent correlation between various blood glucose indicators (including admission blood glucose [ABG], admission hyperglycemia [AH], fasting blood glucose [FBG] on the next day after admission, and stress hyperglycemia ratio [SHR]) and END and poor outcome after intravenous thrombolysis. Results:A total of 319 patients with AIS were enrolled, including 126 (39.5%) had AH, 67 (21.0%) had END, and 85 (26.6%) had poor outcomes at 3 months after onset. Multivariate logistic regression analysis showed that after adjusting for confounding factors, ABG (odds ratio [ OR] 1.188, 95% confidence interval [ CI] 1.105-1.278; P<0.001), AH ( OR 4.246, 95% CI 2.291-7.869; P<0.001), FBG ( OR 1.272, 95% CI 1.139-1.420; P<0.001), and SHR ( OR 2.559, 95% CI 1.192-5.664; P=0.016) were independently associated with END, while all blood glucose indicators were not independently associated with the poor outcomes at 3 months after onset. Conclusion:Higher blood glucose is independently associated with END after intravenous thrombolysis in patients with AIS, but not with the outcomes at 3 months after onset.

ABSTRACT

ABSTRACT Background: The physiological stress of critically ill patients can trigger several complications, including digestive bleeding due to stress ulcers (DBSU). The use of acid secretion suppressants to reduce their incidence has become widely used, but with the current understanding of the risks of these drugs, their use, as prophylaxis in critically ill patients, is limited to the patients with established risk factors. Aim: To determine the appropriateness of the use of prophylaxis for stress ulcer bleeding in acutely ill patients admitted to intensive care units and to analyze the association of risk factors with adherence to the prophylaxis guideline. Methods: Retrospective, analytical study carried out in three general adult intensive care units. Electronic medical records were analyzed for epidemiological data, risk factors for DBSU, use of stress ulcer prophylaxis, occurrence of any digestive bleeding and confirmed DBSU. The daily analysis of risk factors and prophylaxis use were in accordance with criteria based on the Guidelines of the Portuguese Society of Intensive Care for stress ulcer prophylaxis. Results: One hundred and five patients were included. Of the patient days with the opportunity to prescribe prophylaxis, compliance was observed in 95.1%. Of the prescription days, 82.35% were considered to be of appropriate use. Overt digestive bleeding occurred in 3.81% of those included. The occurrence of confirmed DBSU was identified at 0.95%. Multivariate analysis by logistic regression did not identify risk factors independently associated with adherence to the guideline, but identified risk factors with a negative association, which were spinal cord injury (OR 0.02 p <0.01) and shock (OR 0.36 p=0.024). Conclusion: The present study showed a high rate of adherence to stress ulcer prophylaxis, but with inappropriate use still significant. In the indication of prophylaxis, attention should be paid to patients with spinal cord injury and in shock.

ABSTRACT

ABSTRACT Objective To verify the prevalence and factors associated with the presence of stress in students of a Speech, Language, and Hearing Sciences course. Methods 105 students participated, over 18 years old, of both sexes, enrolled in the Speech, Language and Hearing Sciences course at a Federal University. Students who were not active during participation in the research were excluded. All respond to a sociodemographic questionnaire and Lipp's Stress Symptoms Inventory, which identifies the symptoms of stress, as well as their s—pecific phase and the predominant symptoms. The data received statistical treatment being stipulated the significance level of 5%. Results 77.1% showed results suggestive of stress, of which 77.7% were in the "resistance" phase, 18.5% in the "almost exhaustion"; 2.5% in the "alarm"; and 1.2% in the "exhaustion". As for the symptomatology, 64.2% had a predominance of psychological symptoms, 18.5% physical, and 17.3% physical and psychological. The statistical analysis showed a significantly higher presence of stress among women and among those who do not work, and a relationship between stress and the study year. Conclusion A high number of Speech, Language and Hearing Sciences students with stress, was identified, with the "resistance" phase as the most frequent and the prevalence of psychological symptoms. The factors associated with stress were gender, which presented greater damages among women, the fact that students do not engage in paid work, and the year of the course they were enrolled. The first year had the lowest prevalence of stress, while the last year had the highest result.

ABSTRACT

Introduction: Western studies show that chronic stress is one of the major contributors to the progression of chronic diseases, including diabetes mellitus and cardiovascular diseases, increasing the risk of mortality experienced by some stressful professions. In Chinese medicine, the Diagnostics of Traditional Chinese Medicine (TCM) for chronic stress have not yet been established and developing a screening instrument can assist in determining conditions that are treatable prematurely, before the onset of a disease. Objective: Construction and content validation of an Inventory for Tracking TCM Diagnostics applied to chronic stress. Method: This is a methodological study. In the first stage, a database of 805 participants with medium, high and very high stress levels, according to the Vasconcellos Stress Symptoms List (SSL), was subjected to an exploratory factor analysis (EFA) to obtain the main symptoms, to which possible TCM diagnoses have been associated. These were evaluated by a Committee of seven judges and the TCM symptoms and diagnoses were validated by the Content Validity Index (CVI). In the second stage, to the symptoms of the SSL, other diagnoses of TCM were added by the literature, for the construction of the Inventory and submitted to a Committee of five judges who validated the symptoms by the content validity ratio (Content Validity Ratio), according to criteria of clarity, representativeness and pertinence. Results: EFA resulted in 15 main factors, corresponding to 57.3% of the items\' variance. When performing the Reliability test (Cronbach\'s alpha) for each of the factors, seven were considered reliable. From these factors, three main domains were associated: General deficiency (Qi, Yin, Yang, Xue); Shen\'s and Liver Disorder. Thirty-seven symptoms of SSL, related to the three factors, were submitted to the Committee that validated 18 diagnoses of TCM in three evaluation rounds. In the second stage, the Inventory was built with 55 general items (signs and symptoms), five more only for women and two more only for men, totaling 62 items, which were validated by the Committee. The CVR-Content Validity Ratio for each item obtained a ratio equal to one, which represents 100% agreement. To recognize the diagnoses of TCM, from the application of the Inventory, a synoptic table was adapted based on the TCM literature, of the main diagnoses and related signs and symptoms. Conclusion: The Inventory built has a total of 55 items, of which 38 are physical and 17 emotional symptoms and five symptoms for women only and two for men only. These determine 18 diagnoses of TCM, which are mostly related to Deficiencies, Shen and Liver Disorders. Contributions: The Inventory can contribute to clinical practice as an instrument for detecting TCM diagnoses in individuals with chronic stress and different morbidity, as an aid for beginning students and TCM professionals, and can be used by Nursing and by all health professions, that have acupuncture and associated practices as specialties.

ABSTRACT

Abstract Background: Stress is a state of threat to the balance of the organism, which can cause biological and psychological changes. In hypertensive patients, stress can interfere with blood pressure levels, influence on food choices and neglect of the diet. Objective: This study aims to describe the relationship between stress and dietary intake of hypertensive patients. Methods: A transversal study was carried out at the Arterial Hypertension Clinic of the Cardiology Institute of Rio Grande do Sul, Brazil. The participants were aged ≥ 18 years and hypertensive. Blood pressure, food consumption and anthropometric measurements were collected. The variables related to stress were evaluated by the Lipp's Stress Symptoms Inventory (LSSI) for adults. Significance level of 5% has been considered for all analyzed data. Results: The number of participants was 100. There was a higher prevalence of the female sex (67%), the mean age of the study population was 55.87 ± 12.55 years. Among the participants, 86% were classified in some of the stress phases, on which 57% were in the resistance phase. It was observed that there was no correlation between the presence of stress (as well as their actions), pressure levels and food consumption. The consumption of foods rich in lipids and individuals with a prevalence of psychological symptoms of stress displayed a significant association. Conclusions: Rich in fat dietary has been the first choice in patients with psychological symptoms of stress. Further studies regarding remodeled dietary intake and blood pressure levels in relation to the stress phases are suggested. These findings are important to contribute to the development of prevention and treatment strategies for cardiovascular diseases.

ABSTRACT

ABSTRACT Obesity is defined as a chronic and excessive growth of adipose tissue. It has been associated with a high risk for development and progression of obesity-associated malignancies, while adipokines may mediate this association. Adiponectin is an adipose tissue-derived adipokines, with significant anti-diabetic, anti-inflammatory, anti-atherosclerotic and anti-proliferative properties. Plasma adiponectin levels are decreased in obese individuals, and this feature is closely correlated with development of several metabolic, immunological and neoplastic diseases. Recent studies have shown that prostate cancer patients have lower serum adiponectin levels and decreased expression of adiponectin receptors in tumor tissues, which suggests plasma adiponectin level is a risk factor for prostate cancer. Furthermore, exogenous adiponectin has exhibited therapeutic potential in animal models. In this review, we focus on the potential role of adiponectin and the underlying mechanism of adiponectin in the development and progression of prostate cancer. Exploring the signaling pathways linking adiponectin with tumorigenesis might provide a potential target for therapy.

ABSTRACT

Abstract Background: Prenatal stress may increase risk of developing cardiovascular disorders in adulthood. The cardiotoxic effects of catecholamines are mediated via prolonged adrenergic receptor stimulation and increased oxidative stress upon their degradation by monoamine oxidase A (MAO-A). Objectives: We investigated long-term effects of prenatal stress on β (1, 2, 3) adrenergic receptors and MAO-A gene expression in the hearts of adult rat offspring. Methods: Pregnant rats were exposed to unpredictable mild stress during the third week of gestation. RNA was isolated from left ventricular apex and base of adult offspring. Quantitative PCR was used to measure gene expression in collected ventricular tissue samples. The level of significance was set to p < 0.05. Results: β3 adrenergic receptor mRNA was undetectable in rat left ventricle. β1 adrenergic receptor was the predominantly expressed subtype at the apical and basal left ventricular myocardium in the control females. Male offspring from unstressed mothers displayed higher apical cardiac β1 than β2 adrenergic receptor mRNA levels. However, β1 and β2 adrenergic receptor mRNAs were similarly expressed at the ventricular basal myocardium in males. Unlike males, prenatally stressed females exhibited decreased β1 adrenergic receptor mRNA expression at the apical myocardium. Prenatal stress did not affect cardiac MAO-A gene expression. Conclusions: Collectively, our results show that prenatal stress may have exerted region- and sex-specific β1 and β2 adrenergic receptor expression patterns within the left ventricle.
